It sees Wound Present plus Ambulation Independent.

To the bot, this is an equation that does not balance. It is clinically unlikely that a patient with a severe open wound on their heel is walking without assistive devices or pain restrictions.

The algorithm flags the claim instantly. It triggers an Additional Development Request (ADR), effectively freezing your cash flow.


Why Humans Fail at Data Review


This is not an insult to your QA team. It is a statement on human biology.

Humans are terrible at detecting patterns across large data sets. We get tired. We get bored. We suffer from "alert fatigue."

When a QA manager reviews their twentieth chart of the day, their brain begins to fill in the gaps. They see what they expect to see. They gloss over the contradiction because the narrative note says the patient is "doing well."

The Optimism Bias that plagues your field nurses also affects your back office. We want the chart to be right, so we subconsciously ignore the subtle signs that it is wrong.

The algorithm has no such bias. It has no fatigue. It is ruthless in its objectivity.
